What You'll Do:
- Connects residents and home-based non-residents to appropriate supportive services and assists with transitions to higher levels of care when needed
- Monitors the frailest residents through regular contact and post-hospital/therapy transitional care
- Develops and maintains a resource guide, and produces monthly service reports and annual HUD reporting
- Creates communication tools (newsletter, website, social media, marketing materials) and serves as liaison to resident groups
- Coordinates socialization opportunities, including events, classes, spiritual resources, and vocational/educational programming, to reduce isolation
- Works closely with management to ensure residents meet lease obligations and remain in compliance with HUD requirements
What You'll Need
Required:
- High School Diploma or GED
- 3+ years of experience in services for the elderly, disadvantaged, or social services in healthcare setting
Preferred:
- Bachelor's Degree
- 5+ years of experience in services for the elderly, disadvantaged, or social services in healthcare setting
