Physical Therapist Per Diem

What You'll Do: 

Evaluate residents and build individualized treatment plans
Provide hands-on physical therapy to help residents rebuild mobility, strength, and independence
Document evaluations and treatment progress to support care coordination
Share resident progress and concerns with the care team, and with families when appropriate
Follow PT practice standards and New Jersey licensing regulations

What You'll Need: 

Required:

Graduate of an accredited physical therapy program
Current, active New Jersey PT license
Experience working with adults in a sub-acute and/or home care setting

Preferred:

Familiarity with the Net Health Therapy platform
