Physical Therapist Per Diem

What You'll Do:
- Evaluate residents and build individualized treatment plans
- Provide hands-on physical therapy to help residents rebuild mobility, strength, and independence
- Document evaluations and treatment progress to support care coordination
- Share resident progress and concerns with the care team, and with families when appropriate
- Follow PT practice standards and New Jersey licensing regulations
What You'll Need:
Required:
- Graduate of an accredited physical therapy program
- Current, active New Jersey PT license
- Experience working with adults in a sub-acute and/or home care setting
Preferred:
- Familiarity with the Net Health Therapy platform
Pay and Benefits
