What You'll Do
- Build relationships with residents and community members to assess needs, provide guidance, and connect individuals with appropriate support services
- Maintain ongoing contact with residents who may benefit from additional assistance, including those returning from rehabilitation or hospitalization
- Support residents and families through care transitions by helping identify appropriate housing and service options
- Maintain resource directories, track program outcomes, and complete required compliance, funding, and regulatory reports
- Develop and coordinate educational, wellness, recreational, and enrichment opportunities that promote resident engagement
- Create resident communications, including newsletters and community updates, while serving as a liaison to resident committees and groups
- Collaborate with community leadership to support resident eligibility, documentation, and compliance with lease and housing program requirements
What You'll Need
Required:
- High school diploma or GED
- 3 years of experience serving older adults or people with disabilities, or in social services in a healthcare setting
Preferred:
- Bachelor's degree

Schedule
Full-time, Monday through Friday, 8:30am to 5:00pm.
