What you'll do

Evaluate residents and build individualized treatment plans
Provide hands-on therapy to improve mobility, strength, and daily function
Document evaluations and progress notes accurately and on time
Coordinate with nursing and the care team on resident goals
Update physicians and families on progress as needed
Follow state PT practice standards and infection control protocols

What you'll need

Required:

Graduate of an accredited physical therapy program
Active Physical Therapist license in New Jersey

Preferred:

Experience working with adults in a sub-acute and/or home care setting
Familiarity with the Net Health Therapy platform

Schedule and pay

$46.38 to $52.18 per hour, based on experience
$3,000 sign-on bonus, paid per facility policy
Part-time, daytime hours with flexible days, and the option to pick up weekend hours
