About the Community
The Shores is a community serving residents in assisted living, memory support, skilled nursing, and rehabilitation. Our Assisted Living team plays an essential role in helping residents stay healthy, safe, and as independent as possible every day.

What You'll Do: 

Lead and support Resident Care Assistants to help ensure excellent resident care each day
Complete resident assessments and individualized service plans in partnership with residents, families, and the care team
Monitor resident health, respond to changes in condition, and coordinate appropriate follow-up care
Oversee medication management, physician orders, and clinical documentation to support safe, quality care
Coordinate appointments, healthcare services, and other resident wellness needs
Support resident transitions, including move-ins and changes in care levels, while ensuring clear communication and documentation
Collaborate with interdisciplinary team members to promote resident well-being, independence, and quality of life

What You’ll Need
Required:

Active New Jersey RN license
Previous nursing experience

Preferred:

Experience in a long-term care setting
Strong interpersonal and communication skills
